The Long Island Health Collaborative serves as the Long Island region’s Population Health Improvement Program. PHIP is a New York State Department of Health grant initiative. LIHC facilitates collaborative efforts in community and population health among county health departments, health and social service community-based organizations, health plans, academic institutions, physician groups/associations, schools and libraries and similar partners. LIHC collects and analyzes primary data and looks at secondary data sets to help all providers better address the region’s most pressing health needs in a strategically-informed way. LIHC assists hospitals and county health departments with their community health needs assessment reports and community service plans.
